Two ways
to be a patient.

You can be a patient at Doxen Health through an insurance plan we accept, or, if you don’t have coverage, through our Annual Care Plan. Here’s how each one works.

How It Works

Two ways to be a patient here

Doxen Health is a hybrid practice. We bill insurance for patients who carry it, and we offer a flat-rate Annual Care Plan for patients who do not.

Path One

Insurance

We bill your plan directly for covered primary care visits, labs, and services, in the standard way, with no membership fee.

  • We check your insurance eligibility before your visit whenever possible
  • Your plan’s copay, deductible, or coinsurance applies based on your benefits
  • Medicare patients receive an Advance Beneficiary Notice when a service may not be covered
  • We help coordinate prior authorizations when your plan requires them

Annual Care Plan

Self-pay · No insurance billing

A flat annual rate for patients without insurance coverage, payable yearly or monthly, with no per-visit fees.

$1,499 per year, per adult about $125 a month

Enrolling two or more adults? Every member saves 10%, or $1,349 per year each.

What’s Included
Annual Care Plan

Your primary care, one annual rate.

The Annual Care Plan is designed for adults without insurance coverage who want a primary care home. Your plan can be paid yearly or monthly, and your primary care visits at our practice are included.

Your plan includes

  • Sick and wellness visits
  • An annual comprehensive physical
  • Direct provider messaging
  • Telehealth visits, for established patients
  • In-office lab draws as a courtesy, for established patients

What is billed separately. Many patients pair an Annual Care Plan with a lower-cost plan for hospital and specialist coverage. Your plan covers primary care visits at our practice. Laboratory testing, medications, and specialist referrals are billed separately.

We are working to join the networks of the insurance payers below. Participation with each payer is not final until confirmed, the list may change, and not every plan offered by these payers may be included.
