How we practice

We started Doxen Health because we believe primary care works best when there is room to listen, follow through, and plan together. Our practice is designed around that belief, from how we schedule visits to how we stay in touch between them.

Room to talk

We design our visits to leave room for conversation, so there is space for the second question, the one you almost didn't ask. We see that room as part of good primary care, not an extra.

One team, shared standards

Every provider at Doxen Health follows shared clinical standards, and each brings their own experience and style. Your care builds on everything we've learned about your health, your history, and your goals. Our aim is care that stays consistent, whichever provider you see.

Care that fits your schedule

We know a nine-to-five schedule doesn't work for everyone. Online scheduling lets you book whenever it suits you. We also offer extended hours on select days because we know care has to fit around work, family, and everything else.

Clear answers, shared decisions

Lab numbers, diagnoses, and treatment options can be hard to make sense of. We take the time to explain what they mean for you, talk through the choices, and decide on a plan together. Your goals, preferences, and daily life shape that plan, because the right approach is the one that makes sense for you.

Lovie, the dachshund behind the Doxen Health name

Doxen Health takes its name from Lovie, our dachshund, and the quiet inspiration behind what we're building. Lovie's health hasn't always been simple, and caring for her has shown us that good care means listening closely, asking better questions, and being willing to think beyond the obvious.

That's the kind of care we hope to bring to our practice. When you walk through our door, we want you to feel like someone we're genuinely glad to see. The name is a daily reminder of that intention.

Doxen Health is founded and led by Thomas Falletta, DNP, ANP-BC, a board-certified Adult Nurse Practitioner with a background in pain and spine specialty care. Arizona's full practice authority means you receive care from a licensed, independent clinician: no corporate oversight, no production quotas, no conflicting incentives. Just a provider who chose to build something worth building.

We're based in Prescott Valley and serve patients across Yavapai County. We're building something we'd want for ourselves, and we hope you'll be part of it.

Thomas Falletta, DNP, ANP-BC

Co-Founder · Nurse Practitioner

Thomas is a board-certified Adult Nurse Practitioner and co-founder of Doxen Health. He trained at the University at Buffalo, and was formally educated as a teacher before entering clinical practice, a background that shapes how he relates to patients.

His clinical experience spans primary care and specialty settings, including extensive work in musculoskeletal conditions and complex chronic disease. He practices to current clinical guidelines and updates his approach as the evidence changes.

At Doxen Health, he believes the best outcomes come from informed, collaborative care. Understanding each patient’s goals and health history is the foundation for defining what wellness means to them.

Sarah Falletta, MSN, RN

Co-Founder · Practice Manager

Sarah is a co-founder of Doxen Health and a second-generation Arizonan with deep family roots in Prescott. She has worked in healthcare since 2004, starting at the bedside in orthopedics, neurology, and trauma before moving into intensive care, oncology, hematology and bone marrow transplant, and hospice. Over time her work grew to include case management, quality assurance, health informatics, and executive leadership. That path gave her a clear view of how healthcare works at every level, and where it too often falls short.

Sarah’s informatics and leadership experience shapes how Doxen Health runs. The practice is grounded in evidence-based care and process improvement. In her dual clinical and administrative role, Sarah works with patients from their first visit through ongoing care coordination.

Education has been central to Sarah’s career. She is an Associate Professor at Arizona State University and adjunct faculty at Grand Canyon University.

Sarah co-founded Doxen Health with her husband because she believes healthcare should feel personal, not transactional. Inspired in part by her late grandmother, Joy, she brings a special passion for women’s health and hormone care. She is committed to meeting patients where they are and working alongside them toward lasting wellness.
